Fay Constantine

Social Worker/Team Manager, Greater Manchester Mental Health Trust

Fay qualified as a Social worker in 2011 and began her career in a specialist Mental health Assertive Outreach Team. When these services were disbanded and absorbed into a larger Community Mental Health Team, she witnessed how trauma informed practice and the ability to meet co occurring needs were often lost in underfunded mainstream provision. In 2020, Fay made the pivotal move to a specialist homeless mental health team, initially as a practitioner before progressing to team manager. Over the past three years, she has led the team through significant growth following NHS England funding to establish an inclusion focused service for homeless patients with mental health needs. Fay is deeply passionate about specialist services that deliver high quality health and inclusion care. Her discovery of the Critical Time Intervention model has been transformative, offering a framework that not only protects trauma informed practice but also supports mainstream services to respond more effectively to the complex needs of homeless clients.
