Simon is a freelance, currently working for Northampton Borough Council in Housing and Wellbeing. He has been working in social housing for 30 years and is passionate about social justice, the customer, their community and wellbeing; focusing on delivering the best outcomes for the client.
He is presenting his work with Health, preventing readmission to hospital and homelessness of vulnerable people by building holistic plans around the patient. To date only 5% of those with whom he has been engaged has been readmitted to hospital.
